Application for New Service Connection
About the Service
Any individual who is interested with the services provided by San Marcelino Water District must fill up the application form for water connection. It is the policy of the agency to require the owner of the establishment to apply in person.
Requirement
None - if the applicant is also the owner of the residential building
Special Power of Attorney/ Authorization Letter with VALID I.D. of the concerned parties/Affidavit Duly Subscribed or Notarized - if the owner if the building is out of the country or incapacitated
Copy of Leasehold Contract - for establishment under LGU Property
Availability of the Service
Daily except during Saturdays and Sundays, Holiday
8AM – 12Noon and 1PM – 5PM
